Complete all sections of this form to register into the Te Nehenehenui Everyday Health Plan. To be eligible for cover at no cost in the first year of enrolment you must be registered on the Maniapoto Membership Register and be 65 years or older. If you are iwi registered and under 65 years old you can still register at your own cost.

Once registration is confirmed by Te Nehenehenui, you will be enrolled into the Te Nehenehenui Everyday Health Plan and will receive your welcome pack stating your policy start date (which is the date you can start seeking treatment for eligible claims), your Acceptance certificate and Policy document.
